Survey will be prepared by a qualified Marine Surveyor, prior to each cyclic drydocking, unscheduled General Support or
Depot (Contractor) maintenance, prior to return from loan from other Department of Defense Agencies, prior to activa-
tion from storage, and prior to return from U.S. Army users.
(a) Block 1: Enter the organization designation to which the equipment is assigned and location.
(b) Block 2: Enter the End Item NSN following the printed entry, nomenclature and model. Below the
NSN, enter the nomenclature and model of the Marine End Item being surveyed, Example: Landing Craft Utility, Design
1466, Picket Boat, Steel, 46 ft, Design 4003.
(c) Block 3: Enter hull registration number of the vessel being surveyed, Example: LCU 1486, J-3782,
ST2908.
(d) Block 4a: Enter N/A.
(e) Block 4b:
Enter N/A.
(f) Block 4c:
Enter N/A.
(g) Block 4d:
Enter N/A.
(h) Block 5:
Enter calendar date or dates that the vessel was surveyed.
(i) Block 6:
Enter Marine Condition Survey.
(j) Block 7:
Enter TB 55-1900-201-45/1; Date - date of TB.
(k) Changes to Instruction Block indicated on worksheet.
1. Column (a) will indicate TB 55-1900-201-45/1 paragraph number related to the deficiency en-
countered. Strike out TM and enter TB.
2. Column (b) strike out STATUS, enter OP HRS. The operational hours of the equipment in-
spected will be entered in this column, if known.
3. Column (c) same.
4. Column (d) same.
5. Column (e) strike out "initial when corrected". This column will be utilized to enter the man hours
required by General Support (GS) or Depot (Contractor) to correct the deficiency cited in Column c.
