(i) Block 4a: Enter the total man hours required from the bottom of Column 3.
(j) Block 4b: Enter the General Support and Depot (Contractor) rate per hour used to arrive at the cost
in Block 2 Column c of each section.
(k) Block 4c: Enter the estimated total cost to correct the deficiencies indicated on DA Form 2404. This
total is indicated at the bottom of Block 2 Column d.
(l) Block 4d: Enter the expenditure limit based on the acquisition cost of the vessel indicated in SB 700-
20.
(m) Block 5: This block shall be utilized by the Marine Surveyor to state remarks relevant to the vessel
surveyed.
d. Distribution:
(1) One (1) copy to be retained at the Command Headquarters to which the vessel is assigned.
(2) One (1) copy to be retained aboard the vessel.
(3) One (1) copy to be retained at the depot where vessel is being readied for issue.
(4) One (1) copy to be retained at General Support units, as applicable.
(5) One (1) copy to be forwarded to U.S. Army Troop Support Command, ATTN: AMSTS-MWM, upon re-
quest.
(6) One (1) copy to be retained at the office performing the Marine Condition Survey or report of drydock-
ing.
(7) One (1) copy to USATROSCOM, ATTN: AMSTS-SMM when the cost to repair exceeds the mainte-
nance expenditure limits. Reference TB 43-0000-26.

a. Purpose and Use: This form provides a record on the condition of a vessel' bottom, underwater zinc pro-

tectors, shafting, propellers, struts, shaft bearings and of bottom paint as applied,
b. Use: A report of drydocking, painting and condition of vessel bottom will be prepared after each scheduled
or unscheduled drydocking of U.S. Army Watercraft.
c.  Preparation: AMC Form 1386 shall be prepared by the Marine Surveyor. This report applies to all propelled
and non-propelled watercraft and must be approved by the Marine Surveyor's immediate
