form are self explanatory.
d. Distribution:
(1) One (1) copy to be retained at the Command Headquarters to which the vessel is assigned.
(2) One (1) copy to be retained aboard the vessel.
(3) One (1) copy to be retained at the depot where vessel is being readied for issue.
(4) One (1) copy to be retained at General Support units, as applicable.
(5) One (1) copy to be forwarded to U.S. Army Troop Support and Aviation Materiel Readiness Command,
ATTN: DRSTS-MMM.
(6) One (1) copy to be retained at the office performing the Marine Condition Survey or report of drydock-
ing.
