(2) The Marine Condition Survey Analysis, DA Form is a detailed analysis of estimated repair, replacement,
man hour cost and the estimated man hours required by General Support or Depot (Contractor) to correct the deficien-
cies indicated by the Marine Surveyor on DA Form 2404.
(a) Column 1a thru 1h. (DA Form____, Fig 3-2, p. 113). These columns indicate the sections and sec-
tion numbers, in sequence, which they appear in this Technical Bulletin. Each section in this Technical Bulletin contains
designated paragraph numbers, which are in sequence from paragraph 1 commencing from Chapter I to paragraph 117
contained in Chapter III. As an example Section I, II, III and IV pertaining to Hull Inspection of steel, wood, plastic and
aluminum vessels contains paragraphs 6 through 31. Section V, Deck Inspection, contains paragraphs 32 through 46.
(b) Block 2 Column a: Enter the estimated cost for repair and replacement of components, repair parts
and materials at General Support Maintenance.
(c) Block 2 Column b: Enter the estimated cost for repair and replacement of components, repair parts
and materials at Depot (Contractor).
NOTE
The estimated cost indicated in Column 2a and 2b will be the cost to effect re-
pairs to the deficiencies indicated on DA Form 2404 Column 10a. It should be
noted that the paragraph numbers in Column 10a of DA Form 2404 are desig-
nated paragraphs contained within each section of this TB.
(d) Block 2 Column c: Enter the estimated total cost for man hours required to effect the repairs at
General Support or Depot (Contractor).
(e) Block 2 Column d: Enter the estimated total cost for each section.
(f) Block 3: Enter the estimated man hours required to effect repairs at General Support and/or Depot
(Contractor). This is a total of man hours required to correct the deficiencies indicated in the paragraph numbers within a
section from DA Form 2404.
(g) Block 1, i, Other: Can be utilized as required.
(h) Block 1, j, Total: Enter the total of Block 2 Column a, Block 2 Column b, Block 3 Column c, Block 2
Column d and Column 3.
